GENERAL

The Undersigned understands agrees that the associates, contractors or employees may take photographs or videos for the publicity purposes.

DECLARATION OF EXEMPTION FROM LIABILITY :

I, the undersigned, an adult person, desire to voluntarily participate in the Sky Aero Lounge to experience highest and first in Gujarat, dinning in the sky program.
I, declare to be in a good health and not have/had any medical problems such as Heart condition, High blood pressure, Back trouble, severe dizziness etc. That might endanger safety taking part in High Dine Event. Also, the participants declares that She/He is not to be under the influence of any medication, alcohol, drugs or anything of the kind that might affect His/her comprehension.

Participants has to be minimum 18 Years of age. For participants below 18 Years, Parents/Guardians need to sign the document. Please understand Sky Aero Lounge does not admit children below 13 Years of age.

WAIVER AND RELEASE:

• 1. The undersigned, acknowledge and agree that She/He is aware of the risks involved in the activities and have also been apprised by the conductor through his instructors of the inherent risks and dangers associated with Sky Aero High Dine Lounge.
• 2. Assure that she/He is in proper & good health and physically fit to learn and also participate in the Sky Aero High Dine event and represent that She/He is not suffering from any disease which might remotely either put themselves or the other participants to any risk or danger.
• 3. State the She/He is not suffering from Acrophobia condition in which the individual experiences an intense fear of heights as Sky Aero Lounge activity involves dinning at 150ft up from the ground level.
• 4. Shall not hold the conductor, instructor, employees other participants, owners, lessors or anyone responsible for any injury that might happen during the event.
• 5. Hereby release, discharge and agree not to sue the conductor, instructor, employees, any sponsors, advertisers, other participants and if applicable, owners and lessors of premises on which the activity takes place, from all liability, claims, demands, losses, injuries, damage to property or other damage caused or alleged to be caused on my account in whole or in part by the bonafide errors of the "Releases".
• 6. The undersigned agrees to waive any or all claims that has or may have in the future against Cloud dinning and its insures, employees, officers, directors, contractors, Sub-contractors and associates and the releases from any and all the liability for any loss, damage or injury, including death that the undersigned may suffer, or that the undersigned next of kin may suffer, as the result of the undersigned's participation in High Dinning.

Ihave read this agreement and I fully understand its terms; I also understand that I have given up substantial rights by signing the agreement and have signed it freely and without any enticement or assurance of any nature and intend it to be a complete and unconditional release of all liability to the largest extent allowed by law and agree that if any portion of this agreement is held to be not valid, the balance shall continue in full force and effect.
